I use a No Hands mouse on a daily basis.  In concurrence with the other
comments, the main problem is that it is not very rugged.  It's annoying
having to send it back to get repaired.  But I cannot use a regular mouse
or trackball -- it's too terribly painful.  (By the way, using the foot
mouse for too long a continuous period can hurt my ankle.  However, a
painful foot is preferable to a painful hand.)
