I've heard good things about MailWasher.  It manages to download only the
headers and delete spam before downloading the message bodies.  But, of
course, it only runs under Windows, which is not an option for me.

Also, if the spams actually reach my mail server, it's too late.  Spams
will use up my bandwidth.  This is why I like sneakemail.com.
