I finally got in touch with Bally Total Fitness. They said that a refund for the $90 already paid over the last 3 months was impossible since, with an "ongoing membership" type, you must either write or call to terminate the automatic electronic bill payment program, despite my demonstrating to the membership support person that I (you, actually) had 2 active memberships at the same time. This makes me doubt whether the forms that Chase sends will do any good.
