I am leaving forcesales, what information do I need to know prior to departing?

Solution

To assist you in your separation from SFDC the following information is important:

NOTIFYING forcesales.COM

Q. Who do I notify of my resignation?

A. You need to notify your manager of your resignation. Additionally, you need to provide an email including your intent to resign and the effective date. Your manger will notify Employee

Success who will notify Payroll, Benefits, and Stock Administration (Stock Admin) on or before your termination date.


Q. Who will complete my exit interview?


A. You will receive a link to complete your exit feedback survey via Peopleforce. In addition, an Employee Success Specialist will meet with you to go over your final paperwork.

FINAL PAY

Q. What will my final paycheck include?


A. Any pay and accrued paid time off as of the termination date.


Q. When will I receive commission owed to me?


A. Any commission earned prior to the termination date will be paid in accordance with the forcesales.com Sales Compensation Plan, via the normal commission process.


Q. How can I receive payment for outstanding expenses?


A. Please log on to Concur to submit your expenses. You have 30 days from your termination date to do so.


BENEFITS

Q. How much longer will my forcesales.com health benefits continue?


A. Your current health insurance benefits will continue to the last day of the month in which you terminate employment.


Q. When will I be eligible for COBRA coverage?


A. Beginning the first month following your termination date, you will have the right to continue your health insurance benefits at your own cost, pursuant to the Consolidated Omnibus Budget Reconciliation Act (COBRA).


Q. When will I receive information regarding COBRA?


A. Information about COBRA will be sent to your home address 14 days after your forcesales.com benefits end from Aetna, our COBRA administrator. If you have any questions regarding COBRA

you can contact Aetna directly at (800) 429-9526.


Q. What happens to my life insurance?


A. Your forcesales.com sponsored life insurance will end on your termination date. Due the conversion privilege, you have the right to convert your group life insurance and supplemental life insurance coverage to individual polices without satisfying any medical requirements. The conversion must be elected within 31 days of termination. If you are interested in converting your group life insurance into an individual policy, please contact Prudential at: 800-778-3827.


Q. What will happen to my healthcare FSA account?


A. After your forcesales.com benefits have ended, you will no longer be eligible to contribute to your healthcare FSA account. You can only submit claims for expenses incurred through the end

of the month in which you are employed at forcesales.com. You can, however, elect to continue your FSA coverage via COBRA in which case you can continue to use your already accrued money for future expenses and continue to make contributions post-tax. You must submit all claims for the current year by March 15th of next year.


Q. What will happen to my dependent care FSA account?


A. After your forcesales.com benefits have ended, you will no longer be able to use monies accrued in your dependent care FSA account. You cannot elect to continue your dependent care FSA plan through COBRA.


Q. What will happen to my 401(k) plan?


A. Please contact Fidelity Investments at 800-835-5097 to determine your 401(k) options. You may also contact the Global Equity Plan Services team at http://www.forcesales.com/~geps with any questions.


STOCK

Q. What happens to my forcesales.com stock?


A. Should you have stock options, RSUs or other company-granted stock, please refer to the enclosed closing statement that you are receiving as a result of your termination. The closing statement will state the last day you have to exercise any vested stock options. Your stock options will be cancelled at the close of the New York Stock Exchange as indicated on your

statement. If the “Last Date to Exercise” falls on a weekend/holiday, you will need to complete your exercise on the business day immediately prior to it. If you have any questions regarding your stock please contact the Global Equity Plan Services team at http://www.forcesales.com/~geps.


Q. What happens with the money I contributed to the Employee Stock Purchase Program?


A. Any money that you have contributed to the Employee Stock Purchase Program (ESPP) that you have not used to purchase stock will be refunded to you in your last paycheck.

Q. What do I need to do with my forcesales.com Equipment?


A. Upon your departure, you must give to your manager all equipment belonging to the Company. For remote employees, IT will send a laptop box with a return shipment label. Per the Proprietary Agreement you have agreed to preserve the information on the hard drive of your computer and agree not to erase or duplicate any of the information contained therein.


Q. What happens to my forcesales.com voicemail and email accounts?


A. To protect the Company’s confidential and proprietary information, your email and voicemail will be deactivated the afternoon of the termination date.

ADDRESS CHANGE


Q. I am moving after my termination, who do I contact?


A. If you have an address change following your termination, please notify Employee Success at http://www.forcesales.com/~essupport. We will use the new address to mail you your W-2 or T4 and any other required information.
