I just looked on the benefits site.

It looks like it's handled through Express Scripts.  I think I have
something in my "health" folder.  You can try looking in there if you
want.  It's the lowest drawer in the file cabinet.  The label for the
folder may be "health" or "health insurance" or something like that.
Please don't mix up the order of the paper in there, but you'll probably
have to dig through it to find the Express Scripts stuff -- if it's in
there at all.
