Dear Robert:
We are writing to confirm that a security freeze was placed on the National 
Consumer Telecom & Utilities Exchange,
Inc. (NCTUE) data report on May 29, 2018. The NCTUE is a national, member-owned 
consumer reporting agency
comprised of cable, electricity/power, phone, gas, water and pay TV providers. 
These members, through its
database, exchange information on new-connect requests, defaults, account 
payment history and fraudulent account
activity.
In order to (i) permanently remove the security freeze or (ii) authorize NCTUE 
to release your data report for a
specific party or period of time, you must provide us with a 10 digit personal 
identification number ("PIN"). Your PIN is
1409231620. It is for your personal use only and should not be provided to 
anyone else, including potential creditors
or service providers. If you lose or forget your PIN, you may obtain a new PIN 
by mailing your request via mail,
together with a payment of $0.00 ($0.00 if age 65 or older), if applicable, to:

You may have the security freeze temporarily lifted or permanently removed by 
contacting us at
https://www.exchangeservicecenter.com/freeze, calling us at 1-866-349-5355 or 
mailing your request to the address
above. With either request, you must provide the following personal ID 
information: (1) complete name, including any
suffix (e.g. Jr., Sr., etc.), (2) complete address, (3) Social Security number, 
(4) date of birth, and (5) PIN. Your request
must also include, as applicable, the date range (e.g. March 15 - March 21) for 
the temporary lifting of the freeze or
the proper information regarding the third party who is to receive your data 
report. We cannot complete your request
without all of the foregoing information. Incomplete requests will be returned.
Note: It is recommended that you temporarily lift the security freeze for a 
specific period of time rather than for a
specific party or parties, particularly if you are applying for services such 
as telecom, pay TV or utility. Temporarily
lifting the freeze for a specific party may delay the service application and 
approval process.
Upon receipt of your request, we will provide you with confirmation that your 
request to lift the security freeze for a
specific period of time has been processed, or, if you request that we lift the 
freeze for a specific party, we will issue to
you a one-time use four digit PIN for you to provide to that party that will 
enable same to access your data report. It is
possible that the use of the PIN will delay the application and approval 
process. Therefore, temporarily lifting
the freeze for a specific period of time will provide you greater flexibility.
The charge to place the security freeze on your NCTUE data report is $0.00. The 
charge to temporarily lift the security
freeze for a specific period of time is $0.00. The charge to temporarily lift 
the security freeze for a specific party is
$0.00. The charge to permanently remove the security freeze from your NCTUE 
data report is $0.00. If you are age
65 or older, there is no charge to place the security freeze on your NCTUE data 
report, but there may be a charge of
no more than $0.00 to temporarily lift, permanently remove or replace the 
security freeze.
Please include payment by check or major credit card for the appropriate fees. 
For VISA, MasterCard, Discover or
American Express payment, please include your name as it appears on the card, 
the card number and the expiration
date.
If you are a victim of identity theft and you submit or have previously 
submitted a copy of a valid police report or valid
Department of Motor Vehicles investigative report that alleges a violation of 
Section 530.5 of the Penal Code, no fees
will be charged to place a security freeze on your NCTUE data report, 
temporarily lift the security freeze for a specific
party or specific period of time, to permanently remove the security freeze 
from your NCTUE data report or for a
replacement 10 digit PIN.
We will provide you with written confirmation of the successful completion of 
your security freeze request.
