   *  A photo ID or other identifying information
   *  Your COVID-19 vaccine card if this isn't your first dose. If you do not 
   have your COVID-19 vaccine card, please bring another form of proof of 
   vaccination so that we can ensure you receive the correct vaccine.
   *  This confirmation barcode to expedite your check-in. You can print this 
   email or display it on your phone.

   *  If available, bring your Medi-Cal, Medicare, or other insurance 
   documents. Insurance is not required. You will not be billed, and the 
   vaccine will be given to you at no cost . We collect insurance information 
   for reimbursement by insurance companies.

   reach you.

   who is not able to consent for vaccination, is not accompanied by a legal 
   representative.* For others, completing it ahead of time will speed up the 
   check-in process.

* Please note: If you are scheduling for a minor or another individual who is 
legally unable to provide consent for their vaccination, a person legally 
authorized to make healthcare decisions for the individual must be present at 
the appointment to sign the consent form, or the forms need to be printed and 
signed by the legal representative ahead of time and brought to the 
vaccination appointment. In addition, if the legal representative is not 
present at the appointment, the legal representative must be available by 
phone at the time of the appointment if discussion is needed with the 
decision-maker.

Please arrive 15 minutes before your appointment. Do not arrive more than 30 
minutes early. Early entrance is not allowed as it results in prolonged wait 
times for others.

If this is your second dose, please bring your vaccine card.
