URFPs are attached to the foundation wall using (2) 5"x1/2" Titen HD Zinc Bolts and then attached to the sill plate using (5) 3"x1/4" Structural Screws. This locks the mudsill to the foundation. This is the lower end of the "continuous load path", created during the retrofit, from the foundation up through the wood framing. Mudsill to Floor Joist Anchoring 1 Story $1,475.00 Simpson Strong-Tie framing anchors (approximately 64 - L90) will be installed around the perimeter of the foundation in the crawl space area at a spacing of no more than 32" per Standard Plan Set A / CEBC Chapter A3. This locks the bottom of the flooring system (therefore the floor itself) and all wood framing above the floor, to the mudsill and is the upper half of the continuous load path obtained from the retrofit. Please note once your project is completed and submission for reimbursement payment through EBB has been requested, it generally takes 2 weeks to receive your reimbursement check which will be set up to be paid directly to you. Our Bolt Down The Bay Area Invoice is due to be paid in full within 30 days of completion, so feel free to wait until you receive your payment check from EBB before paying us in full. *Bolt Down The Bay Area will provide home owner a completed DRV Form (Dwelling Retrofit Verification) upon completion of retrofit installation. Plans Preparation and Permit Application - EBB 200 $200.00 Includes drawing project plans, printing, mailing, submitting for the permit, interfacing with the building department and scheduling inspections. (does not include the actual permit fees from the city/county) EBB Clients - Includes all uploading of contractor responsible items to your EBB dashboard (includes all required photos and documents) and also working directly with our EBB contacts to ensure successful program Page 1 of 3 Scope of Work Total completion and reimbursement. *Our service includes providing you full perimeter photos of your completed retrofit installation - These will be available for download for your home files and reference confirming installation is completed as prescribed per the approved plans and EBB requirements. Permit Cost from City/County There will be an additional cost for permit fees from the city/county: This line item is just a place holder at this time. The amount of this line item will be equal to the ACTUAL cost of the permit (receipts for permit fees will be provided to you) which is confirmed once your permit has been issued. Please note, by signing and accepting this proposal you are giving BDtBA approval to obtain the permit on your behalf. If for some reason you decide not to proceed with the project installation but we've already paid for the permit fees, client will still be responsible for the associated incurred costs, including BDtBA admin fees. Bolt Down will clean up and remove all cellulos debris created as a result of your project installation - Cement dust and concrete residue from drilling chipping is left in crawlspace area and is considered industry standard and not harmful. Non-Engineered Projects that have cripple walls over 48" tall will not be issued a DRV (Dwelling Retrofit Verification) form for submitting to insurance companies for possible discounts on earthquake insurance premiums. Any additional work or change orders will be provided in writing and mutually agreed upon before proceeding. BDTBA can exercise the right to cancel this agreement anytime prior to work beginning. The seismic retrofit package is designed to meet or surpass the guidelines set forth by the International Code Council, the Ca Seismic Safety Commission and the Association of Bay Area Governments as defined by " Standard Plan Set A" for the structural strengthening in the crawlspace area. The information in this proposal is based on current practices and standards set fourth in Standard Plan Set A, FEMA Plan Set and CEBC Chapter A3. Although the standards are approved by FEMA, ABAG, CALBO Seismic Safety Committee, EERI-NC, ICC Try-Chapter and SEAONC, no guarantee is made that the use of the information or installation of this program will prevent all losses in all earthquakes. BDBA does not assume any legal liability or responsibility for any losses caused by earthquakes or as a result of applying the information in "Standard Plan A", although this program has proven its effectiveness it is strictly a voluntary preventative measure. BDBA is not liable for any damages caused by a seismic event. Article 5: GENERAL PROVISIONS 1. All work shall be completed in a workmanship like manner and in compliance with all building codes and other applicable laws. 2. To the extent required by law all work shall be performed by individuals duly licensed and authorized by law to perform said work. 3. All change orders shall be in writing and signed by both Owner and Contractor. 4. Contractor warrants it is adequately insured for injury to its employees and others incurring loss or injury as a result of the acts of Contractor or its employees and subcontractors. 5. Contractor agrees to remove all debris and leave the premises in broom clean condition. 6. In the event Owner shall fail to pay any periodic or installment payment due hereunder, Contractor may cease work without breach pending payment or resolution of any dispute. 7. All disputes hereunder shall be resolved by binding arbitration in accordance with the rules of the American Arbitration Association. 8.
