People should be able to approve or disapprove each individual credit
report request and all credit report additions should be able to be
contested using a mediation scheme (a la ebay), all for free.

Credit report agencies should assume liability under all cases, including
both clerical mistakes and security compromises (like the one you
described).  Then, there can be class action lawsuits which will keep them
on their toes.
